NOTICE TO PARENTS:
PLEASE BE AWARE that Maryland Law extends health records privacy to dependent children fourteen (14) years and older.
In most instances (Note 1), your child’s signature will be needed to release Private Health Information (PHI).
A parent who is responsible for a dependent child covered under a health insurance plan may obtain claims information DIRECTLY from the health insurance plan.”
(Note 1)- A statement of legal guardianship will be needed for special situations.
